Reconnect Health Services is a not-for-profit health service organization that provides services for seniors, caregivers, and people living with mental health and addictions concerns.
Who We Are
Reconnect is the amalgamation of a handful of community services organizations, all established in Toronto and all dedicated to supporting the health and wellbeing of their communities. Over the years we have evolved to become a multi-service provider, offering services to seniors and to those experiencing mental health and addictions concerns, as well as developing one of the most comprehensive support programs for caregivers in the GTA.
As we’ve grown, we have learned that no two clients are alike, and the best way to provide service is to be responsive, flexible and innovative. This means we work very hard to ensure we have no wait times for service, that we will reorganize and restructure ourselves to support better care, and that we ultimately want our community to see us as the help they need.
Once a client is in our doors, we commit to doing the navigating, advocating, and coordinating on their behalf. We want to treat people as though they were our family, and coming to us should feel like coming home.

Help to stabilize your health, connect you to the social and healthcare systems, and learn to manage your mental health.
Family and Caregiver
Support Services
FAME supports family members and caregivers who have a loved one experiencing a mental health concern and for those who are caring for someone who is aging.
Health and Wellness Centre
The Reconnect Health and Wellness Centre for Seniors offers social, recreational, and exercise-based programming for older adults and seniors that promotes a healthy, active and connected life.
Clinical Services
Reconnect offers clinical services to support our clients including behavioural, occupational, and mindfulness therapy.
System Supports
Reconnect supports the healthcare system with various projects and services. This includes facilitating procurement for the sector, project management, and performance improvement.

Our Impact
In the last year we served over 7000 people, including:
1,450 who needed mental health and addictions supports
3,360 older adults and seniors through our various groups and services
2,730 caregivers and family members who benefited from our FAME Family and Caregiver Support Services
